A Danger to Herself and Others by Alyssa Sheinmel

Hannah Gold is in a mental institute.  She doesn't really understand why.  She didn't do anything.  They were just playing a game....

Agnes, her roommate for the summer,   fell out their two story window and is in critical condition.  Hannah is so worried about Agnes, but she notices on her paperwork it says she "may be a danger to herself and others."  As she goes through the "program" she starts to remember things about the summer, what happened, and eventually realizes what was real and what was not.

Unreliable narrator like They Were Liars by E. Lockhart.  Mental illness topic like Challenger Deep by Neal Shusterman.

High School recommend.
